POPULYTICS RECEIVES HIGHEST (THREE-YEAR) NCQA RATING IN POPULATION HEALTH FOR SUPPORTING PATIENTS WITH CHRONIC HEALTH CONDITIONS
ALLENTOWN, PA, Jan. 6, 2021 – Populytics, Inc., a Lehigh Valley population health management, health care data analytics firm, and wholly owned subsidiary of Lehigh Valley Health Network (LVHN), has been awarded the three-year Population Health Program Accreditation by the National Committee for Quality Assurance (NCQA). This Accreditation is a recent revision to NCQA’s previous Disease Management Accreditation, reflecting the universal movement from single-condition disease management toward whole-person accountability and health management on a population-wide level.
For Populytics, the Accreditation is focused on the Condition Support program, an effective population health management service within BeneFIT Corporate WellnessSM, the well-being division of Populytics. The program provides health coaching by clinically trained and certified coaches to assist individuals with self-management of diabetes, asthma (adult and pediatric), hypertension, chronic obstructive pulmonary disease, chronic heart failure, and coronary artery disease.

They involve data integration, population assessment, population segmentation, targeted interventions, practitioner support, and measurement and quality improvement, among others. Populytics’ collaborative teams pulled together to meet the requirements of NCQA Accreditation, which is an excellent affirmation of their work and commitment. NCQA Accreditation standards are intended to help organizations achieve the highest level of performance possible while creating an environment of continuous improvement.
